Please print and bring the completed forms to your first visit:

  • HIPPA Signature Page
  • Here are the HIPPA forms to read but you don’t have to print them: Page 1 and Page 2


    If you don’t complete these forms before your appointment, please come 15 minutes early to allow time to fill out the paperwork before your scheduled appointment time. Having the paperwork completed ahead of time will ensure that the most amount of time is spent with your therapist.
